URSULIAK, Tom On December 21, 2014, Tom passed away suddenly at the age of 89 years. Tom will be missed and lovingly remembered by two daughters, Carol (Steve), and Debbie (Bob); three sons, David (Teresa), Steven, and Wes (Pam); eight grandchildren, Susan, Joelle, Jessica, Desiree, Dustin, Trenton, Corey and Laura; two great grandchildren, Isabelle and Daia; extended family and friends. Tom was predeceased by his wife Catherine, parents John and Rose Ursuliak and by grandchild Damon O’Brien.
